 Ages and Stages Questionnaire

The Ages and Stages Questionaire is a baseline screening tool administered by the classroom teacher.  It has 19 different age intervals, ranging from four months to 60 months.  The questionnaire is given within 45 days of enrollment into the program, and then annually on the child’s birth date.  It takes approximately 30 minutes for the classroom teacher to administer this screening.  Children are screened in communication skills, gross motor skills, fine motor skills, problem-solving skills and personal/social skills.

Once the questionnaire is administered, the results are sent to the Wesley House Family Services Inclusion Specialist, who then passes the information along to the parents.  If further evaluation is needed, the Inclusion Specialist will follow up with the Battelle Screening Tool.  The Inclusion Specialist also develops an action plan to help the teacher, parent and child strengthen areas of concern.

Battelle Screening Tool

The Battelle Screening Tool is used as a follow-up to the Ages and Stages Questionnaire if the results show some areas of concern.  The Battelle assesses five areas of development: adaptive behavior, personal/social skills, communication skills, gross and fine motor ability and cognitive skills.  The Battelle is administered by a qualified Wesley House staff member.  It takes approximately 20-30 minutes of the child’s time and is administered at the child’s program center.

Results are shared with the teacher and the parents within two weeks of completion.  If the results show areas of concern, the parents are given a letter of referral to either Early Steps (http://www.cms-kids.com/) or Child Find (http://www.paec.org/fdlrsweb/childfind.htm) for further evaluation.

Creative Curriculum

The Creative Curriculum Assessment is administered twice per year by the classroom teacher.  A pre-assessment is done in September-November and a post-assessment is done in March-May.  The Creative Curriculum Assessment tool is based on the Creative Curriculum model of teaching children.  The main goals of this method are a focus on social/emotional development, physical development, cognitive development and language development.  The assessment tool evaluates these four areas of development in children, and includes resources to give teachers the skills to work with the children on these developmental areas.

Results are shared with parents within two weeks of completion.  The results help the child’s teacher develop lesson plans and/or strategies that will enable all the children to learn the skills they need to prepare them for kindergarten.  Strategies are also created to help individual children work on specific areas.  All screenings and assessments of children require parental consent.
