Wesley House Family Services is a member of the Monroe County Interagency Transition Work Group comprised of representatives from various agencies collaborating to provide a complete continuum of family centered, coordinated services to all children birth through kindergarten with disabilities or identified as at risk of developmental delays.
As a member of the Interagency Transition Work Group, Wesley House Family Services is responsible for the screening and assessment of children birth through 5 years of age who are receiving school readiness funds. These screenings and assessments are administered in order to measure each child’s developmental progress and offer services and resources for any children at risk for developmental delays.
